Abstract: This article presents the development of the elderly's quality of life in health dimensions with the Elderly Declaration of 5 D (5 good aspects) of Baan Song Hong, Huai Khwang Subdistrict, Kamphaeng Saen District. Nakhon Pathom Province. The researchers use a guideline for lesson transcription and assessment of the success of the area which include context, formation of management points, development of operating mechanisms, success and results and success factors. The data was collected by in-depth interviews and was analyzed by examining the three pillars of data, which include project operators, target audience, and project followers. It can be concluded that the formation of management points includes important characteristics of the working group as follows: 1) having a shared common goal 2) having a clear goal 3) having a commitment 4) having the best performance 5) aiming for mutual success 6) having good human relations 7) participating in teamwork 8) having development of an operational mechanism. There is a main working group team from Ban Song Hong Sub-District Health Promoting Hospital, that has an action plan with a clear role and duty. There is also a follow-up to report the results at the meetings. The result reported was that the elderly successfully had health promotion behaviors; not falling, not being forgetful, not being depressed, and having good appetite. The participants who have passed the criteria in all 4 aspects are 79 people out of 94 people, representing 84%. The most successful activity is the one that reflects the results throughout the project. The result from each phase of the project implementation would be presented to those involved and are network partners in the area such as sub-district administrative organizations, schools and temples. They are invited to a meeting and listen to presentations of successes that occur in each phase. It was found that inviting network partners in the area could understand better when they took part in guidelines presentation with provided photographs in the field. So the network partners could see the real picture of community-driven health improvements in the area and achieve the results as intended.
