Abstract: The objective of this research to develop a health self-care model for elderly as diabetes disease. Using research and development model was divided into 4 phases: Phase 1, the study of basic information and related theoretical concepts. Phase 2 Development of a self- care model, Phase III Trial of the Health Self- Care Model, and phase 4, the assessment of the health self- care model of the elderly as diabetes disease. The key informants groups and experimental group include: health workers in charge of diabetes work, elderly caregivers, and elderly with diabetes sum all of them 52 people. The research instruments were knowledge test, the efficacy test, behavior measurement form self- care and satisfaction assessment form. The data were analyzed by determining frequency, percentage, mean, standard deviation, and paired sample t-test. The results found that: The health self- care model of the elderly with diabetes had innovative follow organizing learning activities based on Bandura’s self- efficacy theory and the self- care concept of Orem with 2 innovations i. e. 1. Two friends fight disease, including grouping 2 people with diabetes, an activity is exchange of knowledge, and encourage each other by making video clips, messages, or taking picture of food eaten every morning post the Line group and 2. Pinto- jai innovative was a social support activity from health workers and caregivers of the elderly with diabetes by using Tik Tok program to post video clips of food and self-care for people with diabetes and words of encouragement and then the results of assessing the quality of the health self- care model of the elderly with diabetes found that: when comparing the knowledge, perception and health self- care behaviors of the elderly with diabetes, it was found that the elderly with diabetes had knowledge, awareness and self- care behaviors in post- activity health. It was significantly higher than before the activity ( P <. 05) . The results of the blood glucose test were significantly lower than before the activity ( P <. 05) , and the satisfaction of the elderly with diabetes on the activity pattern was at a high level, accounted for 90.00 percent.
