Abstract: This study employed a quantitative research approach using a one-group pretest–posttest quasi-experimental design. The objectives were to: (1) examine the demographic characteristics of patients with type 2 diabetes mellitus participating in the program; (2) assess changes in blood glucose levels before and after participation in the program; (3) assess changes in blood pressure before and after participation in the program; (4) evaluate patients' knowledge regarding health care and self-management; and (5) assess patients' satisfaction with the Diabetes Mellitus Type 2 and Hypertension Complications Prevention and Reduction Program. The study sample consisted of 30 patients with type 2 diabetes mellitus receiving services at the Urban Community Health Center, Nakhon Pathom Rajabhat University, in 2024. Participants were selected using purposive sampling. The research instruments included a demographic and medical record form, a knowledge questionnaire, and a satisfaction questionnaire. Data were analyzed using descriptive statistics, including frequency, percentage, mean, and standard deviation. The findings revealed that most participants were female (66.7%), aged below 60 years (86.7%), married (76.7%), and had completed primary school education (83.3%). Regarding clinical outcomes, 80.0% of the participants had elevated blood glucose levels both before and after the program. The proportion of participants with elevated blood pressure increased from 66.7% before the intervention to 80.0% at the 12-week follow-up. Most participants demonstrated a high level of health-care knowledge both before and after the intervention (93.3%). Overall satisfaction with the program was also at a high level (93.3%). Among the satisfaction domains, the highest mean score was found for the speakers/instructors (x ̅ = 4.62), followed by food and refreshments (x ̅ = 4.56) and the training venue (x ̅ = 3.89). The findings indicate that the program effectively enhanced participants' health awareness and achieved a high level of satisfaction. However, no apparent improvement was observed in clinical outcomes, particularly blood glucose and blood pressure levels. Therefore, continuous follow-up, individualized counseling, and sustained health behavior promotion should be incorporated into future interventions to improve disease control and reduce long-term complications among patients with type 2 diabetes mellitus and hypertension.
