Abstract: This participatory action research aimed to 1) study the community participation 2) develop a model for community participation and 3) study the effectiveness of the community participation in selfcare of uncontrolled diabetes patients Moo 6, Ban Thamaprao, Tambon Khaokob, Amphoe Huaiyot, Changwat Trang. The samples of study were 46 stakeholders and uncontrolled diabetes patients, 14 samples. The tools used in this research contain: community participation model had period is 8 weeks, and questionnaire. Data were analyzed by frequency, percentage, mean, standard deviation and Wilcoxon-signed rank test. The results showed that 1) The overall community participation was at a high level (x̅ = 3.93±0.48). 2) The model of community participation consisted of 4 steps: (1) participation in decision-making; public forum and focus group discussion, ( 2) participate in operations by educational, food-conscious activities, health-conscious with food model, experience exchange, exercise, role model, and health diary, ( 3) participatory in gain benefits, increase gaining knowledge, exercising, having better self-care behaviors and ( 4) participating in the evaluation by tracking exercise and assessing the progress of the health diary. 3) Effectiveness of the community participation model found that knowledge, self-care behaviors and selfefficacy after using the model was higher than before using the model and blood glucose after using the model decreased than before using the model at statistically significant level of 0.05.
