The 4 phases were: 1) preparation to survey about the problem and observation of ICU-med nurses’ behavior, research on the literature review and evidencebased related to VAP, definition of the research objective, as well as, determining indicator of nursing outcomes; 2) a developed VAP practice guidelines from first phase data, quality content by 3 experts, assess suitability of VAP practice guideline using the AGREED measurement and test at ICU-med with 6 nurses, female medication-2 ward with 5 nurses; 3) correct practice guidelines used at same 2 ward with 30 nurses; and, 4) assessment of the effectiveness of practice guideline, analyzation of the problem and barrier of method, and summary of used and/or not used practice guideline. Conclusion1) The incident rate of VAP from survey and observe in ICU-med in year 2020-2022 showed 6.00, 6.85, 9.38 times/1,000 day per hospital bed, respectively, on which has shown an upward trend that is higher than the KPI of the hospital. 2) observing nurses’ behavior shows that nurses have knowledge but not totally accurate and the same with the guidelines. 3) The VAP practice guidelines which is concise and easy to use, has the evaluation result of, CVI 1.0, that is equivalent of having a high score.
